By creating an account with Oaklin Lane Academy, I acknowledge and agree to the following on behalf of myself and any minor children I enroll:
1. Assumption of Risk. I understand that childhood development activities and classes (in-person and virtual) involve inherent physical and developmental risks. I assume all such risks on behalf of myself and any child I enroll.
2. Release of Liability. I, on behalf of myself, my child(ren), and our heirs, release Oaklin Lane Academy, its affiliates, instructors, and staff from any and all claims, damages, or liabilities arising from participation in Oaklin Lane Academy programs, except those resulting from gross negligence or willful misconduct.
3. Medical Authorization. I authorize Oaklin Lane Academy staff to seek emergency medical care for my child if I cannot be reached. I agree to disclose all known allergies, medical conditions, and accommodations on each child profile.
4. Conduct & Cancellation. I agree to follow program rules and acknowledge that Oaklin Lane Academy reserves the right to remove participants for safety or conduct reasons. Refund policy is communicated separately and may vary by program.
